This form is necessary to help further assist us in processing your warranty claim with Lady Americana Northwest / Sound Sleep Products customer service department located in Sumner, Washington.
Please fill out the following and check each box that applies. Pictures are required no matter what unless Stains form is filled out completely with credit card information.
The mattress has been properly supported with a center support bed frame or solid surface (queen or king size only).
You have read the limited warranty form that was sent with your mattress set.
You have the original purchase receipt from the store where you purchased the set. Photo copy of your sales receipt must accompany this form.
Check to make certain there are NO stains or soiling of any type on the mattress set.
The mattress has not been folded, scraped, or torn.
Check to make sure the "Law Label" or "Do Not Remove" label is intact, and record the following in formation from the "Law Label".

If the information is not on tag send picture of the tag you have.
The mattress has been rotated or turned according to the instructions in the limited warranty.
If the problem is a structural or workmanship defect (ie. protruding spring, missed stitching etc.) please describe in detail.

If the complaint involves an impression or other condition of the sleeping surface, check one of the following.
There is no visual impression, but the sleeping area has softened.
There is an impression in the sleep surface, but the mattress is still comfortable.
There is no visual impression, but we seem to roll together or toward the center of the mattress.
There is a permanent visual impression in the sleep surface, and it is greater than 1 1/2".
Please provide a picture of the defect according to the following guidelines.
STRUCTURAL DEFECT --Take a picture showing the problem as described.
PERMANENT SURFACE IMPRESSION -- Follow the instructions below.
- Allow the mattress to fully recover (10-15 minutes) with out any weight on the surface.
- Lay a straight surface ( broom handle, yardstick, etc.) and then measure from this straight edge to the lowest general surface area (not to the bottom of the stitched impressions)
- Take at least 2 digital pictures of the area with the depression. Please see examples below.
- Take a digital picture of entire surface area.

Correct:
Measure to the lowest part of the general surface area.
If all pictures necessary (surface photo, measurements, or photos showing problem) are not present, your claim will take longer to process than 1 week from receipt.
